If you will be participating in a sports program at CLA (Basketball, Volleyball, Track, Golf, etc.) you must complete the FHSAA Participation Physical.

Please note, the form is good for one year from date of completion. You only need to complete one form even if you are participating in multiple sports.

Download the FHSAA Physical Form, and print the form. Parents, please fill out pages 1 and 2, and a doctor will fill out pages 3 and 4. 

Return completed form to CLA.
